Brand Name: PENCITAL
Strength: 2.25g (2g Piperacillin + 0.25g Tazobactam)
Form: Lyophilized powder for injection
Route of Administration: Intravenous (IV)
Pharmacological Class: Broad-Spectrum Penicillin Antibiotic with β-lactamase Inhibitor
Prescription Status: Prescription-only (Rx)
Overview:
PENCITAL 2.25g Injection is a broad-spectrum antibacterial agent that combines Piperacillin, a potent extended-spectrum penicillin, with Tazobactam, a β-lactamase inhibitor. This combination enhances the antibiotic activity of piperacillin by protecting it from degradation by β-lactamase enzymes produced by resistant bacteria.
It is widely used for treating moderate to severe infections, including hospital-acquired infections, and is particularly effective against Gram-positive, Gram-negative, and anaerobic bacteria.
Mechanism of Action:
- Piperacillin interferes with bacterial cell wall synthesis, leading to cell lysis and death.
- Tazobactam inhibits β-lactamase enzymes that would otherwise break down piperacillin, thereby extending its antibacterial spectrum, especially against resistant strains.
Indications:
PENCITAL 2.25g is indicated for the treatment of:
- Lower respiratory tract infections (e.g., pneumonia)
- Urinary tract infections (UTIs)
- Intra-abdominal infections
- Skin and soft tissue infections
- Gynecological infections
- Septicemia (bloodstream infections)
- Bone and joint infections
- Hospital-acquired and ventilator-associated infections
Dosage and Administration:
- Typical adult dose: 2.25g every 6–8 hours by slow intravenous infusion over 20–30 minutes
- Dosage may be adjusted based on the type and severity of infection, renal function, and patient response
- Not for intramuscular use
- Must be reconstituted and diluted before use
Always to be administered under the supervision of a healthcare professional.
Side Effects:
Common:
- Diarrhea
- Nausea
- Rash
- Injection site reactions
Less common but serious:
- Allergic reactions (anaphylaxis)
- Blood disorders (e.g., leukopenia, thrombocytopenia)
- Hepatic enzyme elevation
- Clostridium difficile-associated diarrhea
Precautions:
- Use with caution in patients with:
- Renal impairment (dose adjustment required)
- History of penicillin allergy or hypersensitivity to β-lactams
- Monitor renal function, liver enzymes, and complete blood counts during prolonged therapy
- Prolonged use may result in superinfections, including fungal overgrowth
Drug Interactions:
- May increase the nephrotoxicity of aminoglycosides
- May reduce efficacy of oral contraceptives
- Concurrent use with anticoagulants may increase bleeding risk
- Should not be mixed with other drugs in the same IV line unless compatibility is confirmed
