Overview:

PENCITAL 4.5g Injection is a potent combination of Piperacillin, an extended-spectrum penicillin, and Tazobactam, a β-lactamase inhibitor. This combination therapy provides a broad-spectrum antibacterial effect, making it highly effective against a wide range of Gram-positive, Gram-negative, and anaerobic bacterial infections. The addition of Tazobactam helps protect Piperacillin from bacterial degradation by β-lactamase enzymes, which can otherwise render many antibiotics ineffective.

PENCITAL 4.5g Injection is typically used in hospital settings for treating severe infections in patients who are unable to take oral antibiotics or in cases where oral antibiotics would not be effective. It is commonly administered via intravenous infusion and is indicated for moderate to severe infections, including respiratory, urinary, intra-abdominal, and bloodstream infections.

Mechanism of Action:

  • Piperacillin works by binding to penicillin-binding proteins (PBPs) in the bacterial cell wall, inhibiting cell wall synthesis and leading to cell lysis.
  • Tazobactam inhibits bacterial β-lactamase enzymes, which normally degrade β-lactam antibiotics like Piperacillin, thereby enhancing the antibacterial activity of Piperacillin against resistant strains.

The combination of these two agents provides extended-spectrum coverage, making PENCITAL effective against a wide array of resistant bacterial infections.

Indications:

PENCITAL 4.5g Injection is indicated for the treatment of infections caused by susceptible organisms, including:

  • Lower respiratory tract infections (e.g., pneumonia)
  • Urinary tract infections (UTIs)
  • Intra-abdominal infections (e.g., peritonitis, abscesses)
  • Skin and soft tissue infections
  • Gynecological infections (e.g., pelvic inflammatory disease)
  • Septicemia (bloodstream infections)
  • Bone and joint infections
  • Hospital-acquired infections and ventilator-associated pneumonia

Dosage and Administration:

  • Typical adult dose: 4.5g every 6–8 hours by slow intravenous infusion over 30 minutes to 1 hour.
  • Renal dose adjustments are required in patients with impaired renal function.
  • Not to be administered intramuscularly.
  • Reconstitution and dilution are required before IV administration.

Always to be administered under medical supervision in a clinical setting.

Side Effects:

Common:

  • Diarrhea
  • Nausea
  • Rash
  • Injection site reactions (e.g., pain, swelling)

Less common but serious:

  • Allergic reactions (anaphylaxis)
  • Blood disorders (e.g., leukopenia, thrombocytopenia)
  • Elevated liver enzymes
  • Clostridium difficile-associated diarrhea
  • Renal impairment

Precautions:

  • Renal function should be monitored regularly, especially in patients with pre-existing kidney conditions.
  • Use cautiously in patients with a history of penicillin allergy or hypersensitivity to β-lactam antibiotics.
  • Caution in pregnant or breastfeeding women; consult healthcare provider for risk assessment.
  • Long-term or high-dose therapy may lead to superinfections, including fungal overgrowth or antibiotic-resistant bacteria.

Drug Interactions:

  • Aminoglycosides: Caution when used together as the combination may increase the risk of nephrotoxicity.
  • Anticoagulants: Co-administration may increase the risk of bleeding.
  • Probenecid: May increase blood levels of Piperacillin and Tazobactam.

Storage Instructions:

  • Store in a cool, dry place at room temperature (15°C to 25°C).
  • Protect from light and moisture.
  • Reconstituted solution should be used immediately or within the recommended time frame, depending on the storage conditions.
  • Keep out of reach of children.

Packaging:

  • Available as a single-use vial containing 4.5g of lyophilized powder for reconstitution and intravenous infusion.
  • The vial is supplied with instructions for reconstitution and dilution.

Key Benefits:

  • Broad-spectrum activity against a wide range of resistant bacterial pathogens
  • Effective for the treatment of severe hospital-acquired infections
  • Enhanced stability due to the inclusion of Tazobactam, providing protection against β-lactamase enzymes
  • Administered in a clinical setting under professional supervision for optimal safety and efficacy
